Nephrolithiasis is the most commonly associated renal disease in patients with IBD. Patients with IBD have a 12% to 28% incidence of nephrolithiasis compared with 5% in the general population.1 Nephrolithiasis tends to be more common in patients with Crohn disease and a history of ileocolonic disease, as in this patient.

Sandra C. Hill (b. 1945) Medical Information

October 2003 Dr. Wendy Humphrey suggested I see an endocrinologist after she discovered lumps in my throat. HT and I met with Dr. Todd Brodie 10/8/03 to discuss surgery to remove my cancerous thyroid. 10/29/03 Dr. Kenneth Cournington performed the thyroidectomy followed by radioactive iodine treatment 6 weeks after surgery. Two  days of isolation. Synthroid medicine treatment for life taking 1 pill a day. Every six months bloodwork and appointment with Dr. Brodie. Yearly ultrasound.

Faith, my youngest sister, has had her parathyroids removed, but doesn’t take Synthroid. Maralane, sister #2, takes Synthroid for a hyperactive thyroid. Aunt Vivian, my father’s sister, had her thyroid removed; Normie, my cousin, has a hypoactive thyroid.

Amyloidosis Al

A routine endoscopy in June 2018, because, since childhood, I have a stomach valve that doesn't close all the time and have reflux, found polyps in my duodenum which showed amyloidosis. With no symptoms at age 75, I began my Mayo Clinic journey. July 30, 2018, I met with Dr. Taimur Sher at the Jacksonville Mayo Clinic and began treatment. After many tests, Dr. Sher prescribed chemo of dexamethasone 20 mg and Velcade once a week which I could receive near home in Naples, Florida at the Florida Cancer Specialists. Dr. Steven Newman at Florida Cancer Specialist joined my amyloidosis team, and with Dr. Sher monitored my once a week thirteen chemo treatments from September 17, 2018 to December 10, 2018. Dr. Sher and Dr. Newman gave me Christmas holidays off from chemo. I had three more chemo treatments January 7, 2019, January 14, 2019 and January 21, 2019 and have no had any chemo treatments since to date May 26, 2021 and just receive monitoring every three months alternating with Dr. Newman in Naples and Dr. Sher in Jacksonville.

Peripheral Neuropathy

December 18, 2018 after thirteen chemo treatments I developed peripheral neuropathy in my lower legs and feet. From December 18, 2018 to the end of May, 2019 I used pain and sleep medication and mindfulness to reduce peripheral neuropathy pain to just "weird nerve sensations." Gabapentin from December 24, 2018 to Hydroco/APAP 325 mg four a day and Zolipidem 10 mg (Ambien) and later Dr. Palmer prescribed custom scripted Amit/Keta/Lido 2/5/5% cream. I went from four Hydroco and day to three to two to one and to on half to none. May 2, 2019 no more ambien. May 27, 2019 no pain pills to date, May 26, 2021.

February 11, 2019, I saw Dr. Scott C. Palmer at the Jacksonville Mayo Clinic. Dr. Palmer accepted the Hydroco and Zolipdem prescriptions from Dr. Newman and added custom scripted Amit/Keta/Lido 2/5/5% cream and physical therapy which I took from April 26, 2019 to May 22, 2019.

Bladder (blood in urine) Dr. Timothy D. Lyon (Jacksonville Mayo Clinic) preformed a cystoscopy, bladder biopsy and fulguration 7/23/2020

July 10, Dr. Timothy D. Lyon for a Cystoscopy at the Jacksonville Mayo Clinic which found "a bulbar urethral stricture that was able to be traversed with gentle pressure from the scope. The prostate was elongated and obstructing with trilobar hyperplasia. Upon entry into the bladder it was examined systematically. There were no papillary masses and no suspicious areas of erythema. A left sided narrow diverticular neck was seen, but this could not be examined due to patient discomfort. Bilateral ureteral orifices were not seen. On retroflexed view, there was a large intravesical protrusion of a median lobe." From Dr. Lyon's 7/10/2020 notes.

Back for more tests and surgery July 22, 2020 tests and July 23, 2020 surgery. Dr. Lyon found, "Findings1. Mild distal bulbar urethral stricture2. Trilobar prostatic hyperplasia with large median lobe3. Left bladder wall diverticulum with indeterminate 5mm erythematous patch posterior wall, biopsied. Procedure: "There were no papillary masses and no areas suspicious for bladder cancer. In the left lateral wall was a relatively narrow necked bladder diverticulum. The interior of this was examined carefully. On the posterior wall of the diverticulum was a small, approximately 5 mm erythematous patch of indeterminate nature. We opted to biopsy this given the patient's history of amyloidosis." Dr. Lyon evaluated and assessed the following comorbidities: #1 Hematuria, #2 Amyloidosis (HCC), #3 Multiple Myeloma Not Having Achieved Remission, (HCC) #4 Preoperative Exam, #5 Hypertension Essential Primary, #6 Hyperlipidemia, #7 Left Anterior Fascicular Block and #8 Neuropathy Peripheral.

I have had six kidney stones. My sister had ten kidney stones. Our parents never had kidney stones.

I have glaucoma and take latanoprost 0.005% drops. I also take omeprazole Dr 20 mg, simvastatin 40 mg and losartan 50 mg.  Dr. Lyon July 22, 2020 prescribed finasteride 5mg and tamsulosin0.4 mg.
